Overview & Step-by-Step Instructions
Note: You will need Adobe Acrobat Reader to proceed with your application
If you don’t have Adobe Acrobat Reader on your computer, please visit the Adobe download page to download the latest version of Acrobat Reader.
Please download and fill out the Partnership Application Form below to submit your application to CFCSD. Ensure that you attach all documents, including a 500 word spiritual autobiography covering your evolving relationship with Jesus Christ, attached as a separate document (½ to one page). By typing your name in the signature block, you agree to the information that you have provided.
Overview:
i) Click on the Partnership Application Form link at bottom of this screen
ii) Download the PDF
iii) Fill in the form
iv) Click [Submit Application] button to send application, and remember to attach required documentation
In Detail:
1. Click on the Partnership Application Form link at bottom of this screen
a) This will open the Partnership Application in your browser
b) Do not fill in the Partnership Application yet
2. Click on the Download icon (near top right of window)
a) In downloading and saving the file, rename it to yourname.pdf – the pdf file extension will be appended automatically for you
b) In some browsers such as Firefox, this may trigger an automatic download, in which case you should find and rename the PDF file manually
3. Save it to your computer
4. Open the Partnership Application Form in Adobe Acrobat Reader
5. Fill in the fields as appropriate
6. Click on [Submit Application] button at bottom of second page
a) Dialog boxes will pop up, allowing you to use gmail or other email accounts
7. If not already done by your system, you should now attach your application
8. Attach your educational certificates and spiritual autobiography
9. Send the email with all attachments to membership@christianspiritualdirectors.ca
